Spirit of AAMI Award Application

This award recognizes the outstanding contributions of an AAMI member in volunteer efforts within the association.

Note: Nominees may not be self-nominated for this award. Individuals may be nominated in consecutive years, but must have an original nomination submitted each time.

The prize consists of a plaque to be presented at the AAMI Exchange and a $1,000 check.

Entries for the Spirit of AAMI Award must include:

• Nominator’s cover letter that provides a rationale statement describing the nominee’s commitment to AAMI, and how the nominee has advanced AAMI’s mission and strategic goals, as well as ways in which the nominee has helped bring together diverse stakeholders to forge consensus and improve patient safety.
• This completed Spirit of AAMI Award nomination form.
• A current CV/Resume for the nominee
• Two or more letters of recommendation from the past year supporting this nomination from supervisors, colleagues, and industry partners.
