2. Call us. Call 1-877-249-8226 with credit card information. Or call our local number 240-646-7031.
3. Mail your order to us

Mail completed order form (pdf) and payment/purchase order information to:

4. Fax Us Fax completed order form (pdf) to with credit card to: 240-396-5781

Purchase Orders fax or mail the order form (pdf) and be sure to include the following information.

  • Bill to address
  • Ship to address
  • Name and address of recipient
  • AAMI ID# (required for member discount items)
  • Purchase Order number
  • Product code and quantity ordered
  • Shipping costs
