2. Then choose how to join...

  • Call AAMI today! Call 1-800-332-2264 ext. 214 and a membership representative will take your information over the phone.
  • ONLINE: The online form can be used to pay your membership dues if you are applying for individual membership. If you are applying for institution, corporate, new professional, or student membership — you must submit an application via fax or mail using the PDF forms provided below
  • FAX your membership application form to AAMI. Select the appropriate application form (pdf), complete, and fax to 703-525-1424.

  • Send your membership application form to the address below. Send your completed application with payment information to:
