Sterilization process failures can occur for any number of reasons: a malfunctioning sterilizer; user errors (e.g., incorrect packaging or loading procedures, incorrect cycle selection), poor steam quality, and others. If sterilization process failures are not addressed properly, potentially nonsterile items might be used on patients, placing them at risk.
