The Infusion Systems Steering Committee's Charter is to:

 
  1. Set and monitor infusion safety goals
  2. formation and oversight of working groups to advance infusion safety
  3. Knowledge sharing
Safety statement

There are numerous adverse and near-miss events related to infusion delivery. Infusions are not always administered to patients per clinical intent. Errors and harm, when they occur, are not always detected nor corrected in a timely manner.

What does success look like?
  1. The identification and mitigation of sources of infusion error
  2. A significant reduction (e.g. by 2/3) of specific adverse events related to infusion delivery
Deliverables
  1. Mapping the sources of error
  2. Identification of top errors
  3. Development of plans to reduce errors, including deployment
  4. Measurement of adoption
How will success be measured? Although we recognize the current reporting systems are inexact, we can measure success through items such as:
 
  1. Identification of, and plans in place, to mitigate sources of infusion error
  2. Adoption of error reduction plans
  3. Reduction in frequency or severity of reported ADEs
  4. A change in the type of ADEs reported
Operating mechanisms Steering Committee reports to AAMI Foundation at least annually.
Steering Committee face-to-face meetings at least quarterly.
Working groups report to Steering Committee at least quarterly.
